A phone screening with our Intake Coordinator is the first step in beginning services at Sprout Therapy PDX. This brief call helps us determine whether our outpatient services are a good fit for your needs and to identify appropriate next steps. The screening focuses on availability, insurance coverage, level of care needs, and the types of support you are seeking.
When you request a time for us to call you, you’ll be asked to answer a few brief questions about your therapy history and what you’re looking for. Once your appointment time is confirmed, we’ll send a small set of questionnaires that must be completed prior to your screening.
This screening is not therapy and does not establish care with Sprout Therapy PDX. You will only become a client after completing a full intake assessment and being formally assigned to a clinician. Participation in the screening does not guarantee availability of services, though many clients are able to be matched with a clinician during or shortly after the screening.
Following the screening, one or more of the following may occur:
Referral to individual therapy, therapy groups, medication management, or other internal services
Placement on a waitlist
Referral to external providers or resources if we are not the best fit
Internal Referrals
During the call, we’ll talk about what kind of support might be most helpful for you.
For some clients, this may include therapy alone; for others, it may involve coordinated care with additional providers such as medication management, case management, or peer support. Participation in any service is always optional and based on your needs and preferences.
Important Care Limitations
Sprout Therapy PDX is an outpatient mental health clinic. We are not able to provide:
Crisis intervention or emergency services
24-hour support
Ongoing care for individuals requiring a higher level of care than we offer

Billing and Insurance Information
There is no charge to you for the screening call.
If you are covered by the Oregon Health Plan (OHP/Medicaid), OHP may be billed for this screening service. Information shared during the screening will be documented in your clinical record and protected under applicable privacy laws.
