A phone screening with our Intake Coordinator is the first step of beginning services with us at Sprout Therapy PDX. Once we confirm your appointment time, we will send over some forms that will need to be completed at least 3 business days prior to your screener. During our brief consult call, our Intake Coordinator will determine whether you’re a good candidate for our services and will work to match you with an appropriate clinician at our practice or add you to our waitlist. Please ensure you are available at the phone number provided to us at the start time of your scheduled screening.

-If there are no phone screening appointments available, check back on a later date. There will be no screening appointments between the dates for December 25th and January 3rd. New appointments become available every week.
-Intakes from December screening appointments are most likely to be scheduled in the new year.
-Requests for screening appointments via phone or email will be re-directed to this scheduler.
-Please refer back to this page if our waitlists are closed to the type of therapy you are seeking - information here is updated regularly with clinician availability.
