book your phone screening

A phone screening with our Intake Coordinator is the first step of beginning services with us at Sprout Therapy PDX. Once we confirm your appointment time, we will send over forms that will need to be completed at least 2 business days prior to your screener. During your brief consult call, our Intake Coordinator will determine whether you’re a good candidate for our services and will work to match you with an appropriate clinician at our practice or add you to our waitlist.
